Documented cases of Vit A Deficiency and side effects?

Since Grant stated some examples in the book such as POW soldiers going years without vit A with no side effects etc. Just wondering if there are documented cases of vitamin A deficiency causing; from wikipedia I found:

Vitamin A deficiency is the leading cause of preventable childhood blindness, and is critical to achieving Millennium Development Goal 4 to reduce child mortality.[2] About 250,000 to 500,000 malnourished children in the developing world go blind each year from a deficiency of vitamin A, around half of whom die within a year of becoming blind. The United Nations Special Session on Children in 2002 set a goal of the elimination of VAD by 2010.

VAD is estimated to affect about one-third of children under the age of five around the world.[7] It is estimated to claim the lives of 670,000 children under five annually.[8] Around 250,000–500,000 children in developing countries become blind each year owing to VAD, with the highest prevalence in Southeast Asia and Africa. According to the World Health Organization (WHO), VAD is under control in the United States, but in developing countries, VAD is a significant concern. Globally, 65% of all children aged 6 to 59 months received two doses of vitamin A in 2013, fully protecting them against VAD (80% in the least developed countries).[2]

Not sure if I'm missing something but are these being discounted and if so why?

Because these kids are missing critical nutrition needed for their age. They aren't getting zinc, B vitamins, vitamin C and many others. As well as having no access to sanitary water. I believe POV are grown ups with more resilient bodies. Grant is saying that pinning it down to Vitamin A is misguided. That's what I understood.

It’s because the very definition of VA deficiency is actually that of VA toxicity.

Additionally, for people lacking adequate amounts of dietary protein and fats, and other nutrients, for them even moderate amounts of VA can become toxic. And it is especially so for younger kids.

Dr. Garrett Smith has some excellent posts discrediting this on his blog.  I thought Grant did mention it in one of his books too?  Anyway here are the links but if you search on there, there is more-I also questioned this at the beginning too!

Summarizing as to how I understand it is that there are some.other major deficiencies taking place that many of the studies sidestep (like protein deficiencies) and possibly because of the profits from megadose Vitamin A supplementing.

From what I've read, pork lard is commonly consumed in those areas of the world and can be very toxic especially when coupled with the nutrient deficiences Grant and others spoke off.  Maybe this is a factor also.  Dr. Smith, and I think Grant also, have written about the dangers of pork fat.  There is a a somewhat well circulated story of a canadian boy that went blind eating a seemingly low A diet so the authorities blamed it on low vitamin A consumption.  One of the foods he ate alot of was pork.  Why pork is bad: https://nutritionrestored.com/blog-forum/topic/pork-lard-is-inherently-unhealthy-and-contains-lard-factor-a-form-of-poison-vitamin-a-activity/
