@matrixik I have had a similar experience to you. I don't eat white rice and haven't for 10+ years because I don't tolerate rice in any form. The only grain I am able to eat is oats and I have only eaten oats for short periods of time because they seem to cause me other issues. My diet has ample thiamine in it and yet I still become thiamine deficient very quickly if I don't take additional thiamine.

There were some studies that showed accutane severely depletes thiamine activity.

Sometimes, I think we focus on the wrong organ here. Many think liver, liver. But are we really a bunch of people whose kidneys are silently suffering?
I think I have many signs of low b-vities. One of my kidneys hydrated for no apparent reason during one of my pregnancies. One of my parents is in stage 4 kidney failure.
I learned from the vet and pet owner sites that cats with kidney disease (K/D) deplete their b-vities through increased urination.
My K/D cat is doing good, seems to be "in remission"... .one of the things the vet did was give 2 shots: B spectrum, and B12.
My cat was on a carnivore diet, so you wouldn't THINK his b-vities would be low...(there was no rice (white or otherwise) for this dude, LOL)
