As a substrate for CYP3A4, Ivermectin reduces the amount of CYP3A4 available to metabolize other drugs which use the CYP3A4 pathway, which is about 50% of all drugs. It is also used for some foods, notably grapefruit. There is a small percentage of the population (about 5% more or less) that has a deficient CYP3A4 enzyme (CYP3A4 allele *22) and thus can only produce reduced amounts of this critical liver enzyme. For these folks, any CYP3A4 substrate or inhibitor must be used with caution, and in low doses, as the expected metabolism is "slow" and the products build up and can cause side effects. I'm a believer in IVM (and HCQ) but for me personally, my documented *22 allele makes me look for alternatives.

BTW Paxlovid is a direct inhibitor of CYP3A4 and that is why persons taking certain medications, especially heart meds, must discontinue or reduce these meds while taking Paxlovid. I can't imagine why this drug is given out so loosely; it would seem to require medical supervision and be counter-indicated for those with sensitivities to common meds.
